Survey Excellence

Since its inception in 1995, VistaCare has been uniquely focused on meeting the needs of its patients and their families through palliative care nursing and consistently scores at or above the national average for family satisfaction. Over the years, we have remained true to the principles set forth by our founding nurses and continued to practice a philosophy of comprehensive palliative care. This hospice philosophy provides for the admission and care of all patients eligible for palliative care nursing, regardless of the complexity of their needs, and leads to heightened satisfaction among patients, families, and referral sources, as it removes barriers to access and provides greater choice and personal freedom.

Recognizing that the Company’s most valued resource is its employees, VistaCare also maintains and continually updates a comprehensive library of educational and training resources. These palliative care resources are designed to keep VistaCare staff current on the latest hospice and palliative care modalities, while also ensuring an environment that encourages personal and professional growth and development. VistaCare’s interdisciplinary hospice team includes nearly 1,000 nurses and over 100 medical directors, more than 40% of which are board-certified in hospice and palliative care.

Once a patient is admitted to our services, an individualized plan of palliative care is established by the Interdisciplinary Group (IDG) based on the patient/family’s goal of care. Goals of care are determined as part of living the VistaCare Promises. As part of developing the plan of care, the team determines the amount of support the patient/family will need from each team member and schedules those visits accordingly. The plan of care is reviewed by the IDG team on a regular basis to determine progress towards the goals of care that were determined by the patient/family. As the goals change or the patient’s condition worsens, the plan of palliative care is updated and scheduled visits by the team are changed based on needs. When the plan of care is changed, the attending and referring physicians are updated on the patient’s progress towards achieving the goals of palliative care.
