At VistaCare, our goal is to meet our patients and families where they are - with the care that's right for them. In many cases, this means bringing our special hospice care to wherever our patients and families call home. But we also recognize that not everyone can remain at home when facing a life-limiting illness. And that's what the VistaCare Center at Carillon is all about.

The VistaCare Center at Carillon is a place of comfort where our patients can find around-the-clock nursing care to manage pain and physical symptoms. And it's a place where our families can find emotional and spiritual support, as well as the peace of mind that comes from knowing your loved one is in good hands.

Why VistaCare Center at Carillon?

Some specific reasons to consider a stay at the VistaCare Center include:

  • If pain or symptoms cannot be controlled effectively at home. Sometimes, a patient requires specialized pain control and short-term symptom management. Our 24-hour nursing and support staff are specially trained and highly skilled in meeting the individualneeds of our hospice patients.
  • If the patient's caregivers are overwhelmed by their caregiving responsibilities.
  • If the patient needs specialized wound management.
  • If the patient needs assistance with transition from the hospital to home or to a long-term care setting.

When you come to the VistaCare Center at Carillon, you can expect:

  • A home-like environment with private rooms and dedicated gathering areas for families.
  • Coordination between your personal physician and our hospice Medical Director, who will oversee the plan of care.
  • Nurses on-site 24 hours a day.
  • Social workers and spiritual care coordinators to care for the emotional and spiritual needs of both patient and family.
  • Provision of medications, medical supplies, and equipment.
  • An environment that welcomes all visitors - 24 hours a day.
  • Ongoing grief counseling and bereavement support for at least 13 months following theloss of a loved one.
